Job Summary and Qualifications

 

Your primary function is providing general clinical and pharmacy services, including supervision of technical staff. You will work with a phenomenal team that cares like family and works together in a collaborative manner.

 

In this role you will:

· Interpret orders and transcribe or verify them accurately and efficiently into the pharmacy computer system.

· Accurately compound and dispense pharmaceuticals.

· Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose.

· Sustain the hospital formulary, minimizing nonformulary procurements, utilizing therapeutic interchanges and promotes rational drug therapy selection.

· Provide accurate and timely medication information to healthcare staff and patients.

· Assist with clinical services such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing and other programs as defined by Pharmacy Leadership.

· Document all clinical activities and interventions accurately and completely.

· Complete notification reports regarding medication errors and adverse drug reactions.

· Effectively supervise pharmacy operation activities and provide support to the whole pharmacy team.

 

Job Requirements:

· B.S. or PharmD from and ACPE accredited School of Pharmacy required

· Minimum of one year of hospital experience is preferred

· Active pharmacist license in good standing with the appropriate State Board of Pharmacy
