Family Health Centers of Southwest Florida- PGY-1 Community Based Pharmacy Residency Program

Residency Special Features: Housing for residents will be provided by FHCSWF (no deductions from stipend for cost of housing). Residents will be based out of Cape Coral, FL, but may spend time limited time staffing in Estero, Lehigh, and Downtown Fort Myers. Family Health Centers of Southwest Florida’s PGY1 Community-Based Pharmacy Residency Program offers a comprehensive outpatient training opportunity at a Federally Qualified Health Center. Residents will have the opportunity to improve outcomes for patients with diabetes, hypertension, depression, cardiovascular diseases, respiratory diseases, and chronic kidney disease, as well as other disease states commonly encountered in primary care practice. Residents will develop essential skills and knowledge in various areas of pharmacy practice over the course of the program, including Pharmacy Leadership and Compliance, 340B Drug Pricing Program and its impact on covered entities, Medication Therapy Management (MTM), Centering Diabetes Care and counseling newly diagnosed diabetic patients, and Managed Care Pharmacy (HEDIS Measures and Quality of Care). Residents at FHCSWF will have the unique opportunity to learn from pharmacists involved in multiple aspects of population health at our centers. Over the course of their appointment, residents will develop and lead a medication reconciliation post-discharge program for patients unable to come to an in-person office visit within the 2 weeks following hospital discharge. Additionally, required projects will encourage residents to develop and implement new pharmacist-led programs (e.g. coumadin clinic, specialty pharmacy designation) and will focus on quality of care improvement.

Site Special Features: Family Health Centers of Southwest Florida is a Federally Qualified Health Center (FQHC) and operates as the safety net of the community providing medical care to underserved populations who may not be able to receive care elsewhere. As a 340B covered entity, FHCSWF can pass on program savings to patients in the form of discounted drugs and fee scales for services that decrease the cost of obtaining care. FHCSWF is accredited by the Accreditation Association for Ambulatory Care Health Care (AAAHC) and provides multiple service lines including adult medical, pediatrics, women’s health, dental, optometry, disease management, and pharmacy.

Position Requirements:

PharmD (or equivalent experience), US Citizen or eligible to work in US, Clear and active Florida Pharmacist License or eligible to license within 30 days of start date.
