Title: Impact of Pharmacy Intern Review of Discharge Medication Reconciliation, Patient Education, and Medication Access Optimization in High Risk Acute Care Patients
Description: We are recruiting for a pharmacy intern position at Ascension St Vincent’s Southside. This will be a temporary position focused on optimizing patient outcomes at or after hospital discharge. Pharmacy interns will be trained prior to project start on EHR, proper documentation, education, and resources to help patients with compliance, access, or affordability. This is an excellent opportunity to apply intern’s didactic knowledge and make a difference in patient care.
Schedule: The hours are flexible and negotiable. Preferred availability is 4-10 hours per week 8am-6pm. Hours can be planned and adjusted around other responsibilities.
Payment: The payment is based on Ascension’s HR recommendations. It will be paid on hourly basis.
